WELCOME TO OUR OFFICE.

My name is Dr. Jeremy Rosner, D.C. and let me be the first to congratulate you on taking the first step toward better health and quality of life! We at Wellness 4 Life Chiropractic realize that this may be your fist visit to our office, and our past experiences have shown us that new patients have many unanswered questions on their minds.  Our staff will attempt to do everything possible to make you feel at ease, and to answer any questions that you may have. We are here to serve you.

To ensure your fist visit with us is a pleasant one, here are the procedures you can expect during this visit. Please allow for approximately 1 hour for your first visit.
 
New Patient Forms: Save time during your first appointment and fill out our new patient forms in your PJs at home. Click on the new patient forms link to get started.
NEW PATIENT INFORMATION:
Along with this introduction letter you will find forms to fill out including:

  •     HIPPA Privacy Consent Form
  •     Terms of Acceptance
  •     Personal Health Information Questionnaire
  •     Office Policy on Collection of Fees

CONSULTATION: I will personally discuss your current health status and review your health history with you to determine if you are a candidate for chiropractic treatment.  You will have time to discuss any health questions and I will do my best to alleviate any of your concerns.
EXAMINATION:  Standard physical, orthopedic, neurological, and chiropractic tests will be performed to determine the cause(s) of your problem.

X-RAY: Necessary views may be taken to visualize the location of any spinal problems, reveal any pathologies, and make your chiropractic care more precise.  Before proper care can be rendered, I will study your examination findings.  On your next visit you will see your X-ray, review the doctor’s findings, and get my specific care recommendations.

FUTURE VISITS:   Your fist visit is complete.  Plan to spend about 30 minutes on your next visit to receive the doctor’s report of findings and a chiropractic adjustment.  Details regarding treatment, expectations, prognosis, health care coverage, financial issues, etc. will be discussed in detail before your case is accepted for treatment.
